To Participate:

1. If you are a current patient and would like to refer a friend or a family member for a Free Exam and Consultation, start by typing your name in the box located below.
2. Click the print button and give the printed card to a friend or family member who is in need of dental care. Be sure to tell them to bring the printed card with them to their first appointment.
3. When your friend or family member arrives for their first appointment and hands us the printed card with your name on it, they will be given a Free Examination and you will receive a $50 credit on your account and a surprise gift sent in the mail!
